Volunteer Consent and Release

Please read before signing.

I am aware of the risks related to volunteering for this activity. I agree not to hold the organization, its officers, agents, or employees liable or pursue legal action against them. This waiver releases the organization from liability for injuries sustained on the premises before, during, or after the activity.

By signing this agreement, I agree to hold Covenant Purpose and Restoration Family Center completely harmless, including from financial responsibility for any injuries sustained, regardless of the cause or circumstances.

I will do all in my power to follow staff instructions and all safety requirements, and I will seek clarification when necessary.

I authorize Covenant Purpose and Restoration Family Center to distribute and reproduce photographs or videos of me for portfolio, advertising, marketing, branding, educational, digital, media, and other commercial or non-commercial purposes.
